Case Manager/Home Health Coordinator
As the SHS case manager/home health coordinator at Hollywood,
Madeline Phillips, RN, works with each resident and his or her
family to establish a personalized plan of care and oversees the
delivery of care and services. In addition, Madeline can provide
our clients and their families with information about home health,
assisted living, hospice and other related services.
After beginning her 33-year career in hospital care and
administration, Madeline has turned her focus to long-term and home
health care. She earned her associate's degree in nursing from
Broward Community College and is pursuing her bachelor's degree in
health care administration from Capella University. Call
